Fees and Billing
The fee for the first session is $200 and all subsequent sessions are charged at $180 per hour. The full fee or the copay (based on insurance coverage) is due at the time of service. If clients are using insurance, it is important to be aware of the exact insurance coverage--restrictions, limitations, any necessary precertifications, deductibles, and so on. The best outcome occurs when clients work in tandem with Dr. Weber to insure that there is full awareness of the benefits and limitations of insurance coverage. Please inform Dr. Weber of any changes in insurance coverage.
Other Services
Any letter or report writing is charged at $180 per hour. Phone consultations are also charged at $180 per hour.
Credit Cards
Clients may pay with check, cash, or credit card. If you choose to pay with credit card, please call Dr. Weber's administrative assistant, Terri, at 253-520-3448. Leave a message and she will return your call.
Statements and Insurance Billing
Once every two months, client statements and billings will be sent. If there are any questions about your statement, please call Dr. Weber's administrative assistant, Terri, at 253-520-3448. Clients are fully responsible for the fees for clinical services.
Missed Appointments
A 24 hour notice is required to cancel a scheduled appointment. The full fee will be charged if there is not a 24 hour notice. Clients are fully responsible for the missed appointment fee. Insurance companies do not cover missed appointments.
